Also discussed 7 day date marking parameters needed for pickles made in-house with a hot brine.

Inspector's notes

Observed in the walk-in cooler a container of raw clams being stored without a shellstock tag. Observed many of the shellstock tags kept on-site at the establishment to be missing the final sale date. Per PIC, shellstock tags are typically removed from shellstock box once received and filed away without dating with final sale date. Shellstock tags must be kept with shellstock until sold, dated with final sale date, and maintained in chronological order for 90 days onsite. Discussed shellstock tag protocol with PIC, including keeping the shellstock tag on shellstock containers in the walk-in until final sale date, and then writing the final sale date on the tag before storing them in chronological order. Corrective Action: Correct prior to next routine inspection.

Final construction inspection for a Class 4 10+ E&D permit was conducted with superintendent Adam B. and plan review supervisor by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Only the micro-switch should operate the unit. 4) Switch the hot and cold water at the bar sinks, as discussed. The hot must be on the left. 5) Caulk-in all escutcheon plates and plumbing penetrations, as needed. 6) Elevate the water lines/filters, soda lines at the bar, and wood near the dumpster 6" inches minimum from the flooring. 7) Recaulk the sink fixtures to the walls with silicone, as needed. 8) Ensure the wall tile at the gelato bar is installed smooth and easily cleanable. 9) Apply additional sealer to the exposed brick walls at the bar. All liquids should bead on the finishes. :

The facility is approved to stock and train. Please ensure the following are addressed by the final construction inspection: 1) Provide ASSE 1013 reduced pressure backflow preventers upstream of the glass rinsers. Certify and provide test reports. 2) Ensure the drain lines for the combi-oven are rigid piped and provide with 1" minimum air gaps above the floor sink. 3) Repair the lights in the walk-in cooler unit to ensure 10 foot candles minimum is provided. 4) Continue flashing off the small gaps around the rotisserie oven hood, as discussed. 5) Caulk-in all escutcheon plates, plumbing penetrations, and wall holes throughout the facility, as needed. 6) Provide a clear matte sealer to the exposed brick wall areas, as discussed.
