Inspector's notes
Observed prepackaged salami, cheese, and cracker packs without dates. One of the packages had cheese covered in blue matter. Alerted the manager who was unable to identify the expiration dates of these prepackaged foods (see embargo). The salami, cheese, and cracker packs come in boxes of 4 with the expiration date on the outer packaging. Staff to transfer the expiration date to the individual packs, if sold as individual packs. Staff to routinely monitor foods for adulation and dates. Corrective Action: Corrected at time of inspection.

Observed pre-packaged meals sold from a third party vender in Phoenix without a verifiable permit. Alerted the manager and embargoed these foods, at time of inspection. Facility to refrain from selling these products until the third party is able to get a permit to sell food in Maricopa County. Employees pulled all of these products from the floor at time of inspection and will no longer sell any food from this third party. Finish installing the side panels for the sneeze guard along the front service counter. This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. There is currently copper piping and brass fittings on the downstream side of the RPZ backflow prevention device for the RO system. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass.2. A separate stainless-steel NSF approved food preparation sink is required for the preparation of beverages and for proper rinsing/thawing of food items and food equipment such as blenders. Drain lines must be indirectly plumbed with approved minimum 1-inch air gaps.3. Provide an additional, certified, drinking water safe, RPZ backflow prevention device for the water supply line to the countertop blender rinser.4. Provide a separate ASSE 1022 backflow prevention device for the coffee machine and for the cappuccino machine.5. Provide a hard pipe end piece for the drain line from the cappuccino machine. There must be a minimum 1 in. air-gap between the end of this pipe and the hub drain assembly that it drains into.6. Provide an additional water line, with an angle-stop or ball valve, at the mop sink/ utility area to supply chemical dispensers. Do not use the mop sink faucet. Y- Valves and downstream shut-off devices are not permitted on mop sink faucets.7. The floors in the mop sink room must be repaired and finished to be smooth, non-absorbent and easily cleanable.
