Inspector's notes

Observed raw shrimp and raw marinade shrimp in reach in case unit at temperatures 45-46 degree F. Per PIC they had been put in unit over 4 hours prior. All TCS foods improperly cold held were discarded by PIC at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Final Construction inspection conducted with site superintendent, Steve. Construction completed and approved. Inspection rights and inspection report was emailed to SteveAddress the following items prior to next inspection:1. Train employees not to use toggle switch on the three compartment sink as a shut off valve. Facility is on a recirculating pump. Usage on the toggle switch to turn on the water can cause decrease in required hot water temperature2. Personal items (purses, jackets) to be maintained in lockers, designated areas as in the breakroom3. Ensure employees are air drying utensils prior to storage. Employee at time of inspection nesting wet third pans together. Educated on wash, rinse and sanitizer compartments are each filled with correlating solutions.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are non-refundable. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2.
