Final construction inspection. Construction complete and approved. Superintendent will follow up on the items below before opening:1). Two escutcheons will be installed behind the bag in the box rack.2) Old walk-in cooler condensate drain will be braced to the wall.3). It is advised to seal the trims at the urinal partitions.4). Trash can's with lids will be installed in the ladies rom stalls.5). The drain lines at the bar will be raised so there is a 1inch air gap. 2X6). Super will make sure there is no over splash from the dish washer, shorten the line if there is.7). Return cover will be installed in the ladies room.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Pre-final inspection walk through with the superintendent. Store has hot water, dish wash area is plumbed in and all the refrigeration equipment is on and holding at proper temperatures.Before final superintendent will follow on the items below:1). Backflow valve will be tested and certified upstream of the soda fountain.2). Dual check valves that meet plumbing standard ASSE1022 upstream of any non-carbonated water connected beverage machines.3). Hood over the dishwasher will be caulked in at the wall junctures.4). Door to the exterior will be caulked the frame / wall junctures.5). Coved metal base will be installed in the walk-in.6). Soap and paper towels will be installed at the hand wash sinks.7). Provide drain grates where missing and make sure drain line meets up with drain grate openings.8). At the bar, mount the soda gun drain lines and lead to the floor sink following gravity flow.9). Please seal and caulk the underside of the bar counter.10). Drain line of the dish washer will need to transition into a rigid line and then led over to the floor sink.11). All laser film will be pulled.12). Drain pan on the water heater will be plumbed in.13). Beer cooler will be based out, caulk vertical joint in the front wit wall.14). In the restrooms where schluter is loose from the floor, please seal.15). See if you can install a strip LED light under the bar counter so you can see the glass ware and ice. 20 foot candles is required minimum.16). Provide a rigid drain line for the beer tap drip tray with stand pipe and lead to the floor sink.17). Coke soda station drain line needs to be lifted.Store is okay to stock and train. Inspection walk through of the kitchen and restroom. Dining area, bar and waitress station floor was just placed so did not walk on that floor and that area was not reviewed. The following items were observed for the areas visited.1). Please install come metal coved base along the freezer floor in the walk-in cooler.2). Make sure to lead drain lines along the walls and then over to the floor sink and forward. Like walk-in condensate, ice machine and three compartment sink.3). After laser film is removed, please caulk the hoods at the back wall and the cook line hood also in between, use silver high temperature silicone or silver caulk.4). Metal coved base will need to be installed under the hood.5). I am sure it is planned but please make sure to lower the floor drain in front of the walk-in.6). There is an outlet deep in the wall to the west of the main hood, please bring forward or bridge the gap so there is no opening.7). Escutcheon plate will be installed where missing.8). In the restrooms, where the schluter is not tight with the floor tile, please seal with silicone or grout.Construction is at 80% completion.
