Final Construction inspection conducted with Assistant Site Superintendent and Sit Superintendent. Construction completed and approved. Kitchen does not plan to open until beginning of August 2024.Complete the following prior to routine inspection:1. Replace clean-out cover and seal to wall behind grill.2. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. Inspection report was emailed to Anitoli DelaneyThis establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.No County legal action will result from this inspection.

Equipment inspection conducted with Site Superintendent. At the time of this inspection, this project was about 75% complete. Most equipment is in their assigned location but not connected. The dishwashing machine has not yet arrived. Site Superintendent stated the project shall be completed within the next two weeks.Please ensure the following is completed prior to final construction:1. Ensure hot water temperature at all handwashing sinks is at least 100 degrees F, and at least 110 degrees F at all other sinks.2. Ensure all refrigeration is energized and holding an ambient temperature of 41 degrees F or below.3. Seal gaps in walk-in refrigerator at top of stainless steel coved base, and panel junctures/corners.4. Seal gaps at top of coved base behind stove/grill.5. Seal gaps at wall junctures for handwashing sinks, 3-compartment sink, prep sink, and dishwashing machine.6. Seal escutcheon plates underneath sinks to wall.7. Ensure drain from hot water heater and TMP valve is indirectly drained with a one air gap.8. Ensure condensate line from walk-in refrigerator is indirectly drain with a one inch air gap.9. At dishwashing area, ensure drain line for dump sink and dishwashing machine are separated, and indirectly drained to nearby floor sink with a one inch air gap.10. Ensure drain line for ice machine is indirectly drained to nearby floor sink with a one inch air gap.11. Finish installing ceiling tiles. Ensure tiles are smooth and easily cleanable.12. Finish installing flashing around hood.13. Ensure doors to kitchen have self-closing mechanisms.14. Finish installing food lockers at pass through window.15. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 402-3147 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Plumbing inspection conducted with Site Superintendent At the time of this inspection, this project was about 40% complete. Walked kitchen and verified plumbing stub outs for handwashing sinks, prep sink, 3-compartment sink, mop sink, hot water heater placement, and floor sinks. Discussed relocating handwashing sink near prep sink area so it is not obstructed and within sight of ware washing area. Discussed ensuring 3-compartment sink, prep sink, dishwashing machine, and ice machine drain lines are drained separately and indirectly to nearby floor sinks with one inch air gap. Walk-in refrigerator does not have a condensate line. Ensure water heater is elevated 6 inches above floor, and install splash guards on handwashing sinks within 24 inches of prep/storage areas.Inspection report was emailed to Ryan Holder.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 402-3147 or the inspection request line 602-506-6824 for an equipment/construction inspection between 50 - 75% completion of project or if an additional onsite walk-through is needed.
