Final Construction. Walked with James and Kal. E&D Class 2, 10+ seating.Correct the following items prior to your routine inspection:1) Remove faucet from front service 3-compartment sink so it cannot be used as a sink (all 3-compartment sink ware washing must take place in back room due to outdoor bar requirements). 3-compartment sink has been covered with stainless to be used as a table only. A dish machine is available for front service area2) Seal off gap at left side of walk-in cooler (between walk-in and back wall).Air curtains are not required at this time - Establishment has been set up as an outdoor bar with support area in the back room (3-compartment sink for ware washing, prep, ice machine) sealed off with a reasonably tight-fitting door. Any changes require prior approval from MCESD and changes could require air curtains.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment Inspection. Walked with Kal. Project nearly complete.Correct the following items:1) Have permanent power to establishment.2) Have all refrigeration turned on and holding at 41*F or less.3) Have functional hot water to all sinks (100-110*F) and dish machine (120*F) within 45 seconds.4) Seal all gaps throughout establishment, including under bar counter, saw cut in floor near ice machine, gaps on sides of walk-in cooler, etc.5) Re-paint restroom ceiling a light color.6) Ensure walk-in cooler has adequate lighting.7) Modify the keg urinals so that they will be easier to clean and prevent pooling liquids in the top – weld gaps in rim and provide additional holes in front of top. 8) Provide barrier protection for food service areas. Either install air curtains above all continuous openings (activated by a microswitch when doors are opened) or relocate 3-compartment sink to back room and provide a gasket kit on eliason door to back room. If 3-compartment sink is relocated, ensure it has hot and cold water and drains to the sewer with a 1” air gap at the floor sink.No County legal action will result from this inspection.Report emailed.
