New permit inspection for Surprise Home Base. Walk through conducted. The kitchen is currently not in use. The following items must be addressed before the re-inspection:1). Install split escutcheon plates and mount at waste and water lines where missing.2). Install missing cleanout cover at the hand sink.3). Provide hot water at the food prep sink. A dual outlet angle stop at the hand sink will worm for source.4). Raise the shelve on the prep table and provide 6 inches clearance.5). At the three compartment sink, provide a P-trap where entering the wall. There is a floor sink on the other side of the wall close to 5 feet removed allowing the three compartment sink to remain plumbed direct.6). At the mop sink, flash off the gaps on both sides at the all and caulk all joints, floor and wall.7). Install FRP with edge and corner trims in the splash zone of the mop sink.8). Provide air gaps in the backflow housing part of the eductor of the chemical dispensers at the dish wash gutter and the three compartment sink.9). For the chemical dispenser at the mop sink, or install an air gap like under 8 and provide a dedicated water line or remove the chemical dispenser and patch the holes.Permit issues will be a Boarding Home. regulations pertain to the kitchen/ food service only.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. For further instructions on mail in payments please refer to the printed invoice.Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department.
