Relocating the mopsink5. Backflow for the soda system and the water fed coffee brewers/cappuccino machine6. Replace acoustic tiles with washable ceiling tiles7. Providing a secondary restroom8. Replacing connexes that do not have easy cleanable, durable, non-absorbent and smooth walls to a potential modular building. The following items will be required for approval of the remodel permit:1. Provide indirect plumbed three compartment sink, ice machine2. Provide handwash station in the three compartment sink area3. Provide ASSE 1013 reduced pressure zone backflow preventer for the soda system4. Provide ASSE 1022 dual check valve for the tea machine5. All melamine shelving in the walk in cooler to be replaced with approved surfaces, ie metal or plastic 6. Mopsink to be accessible at all times7. Hot water heater to be a minimum of 30 gallons OR a tankless hot water heater that is sufficient in size to meet peak hot water demands. Tankless hot water heaters are approved on a case by case basis and to have prior approval prior to installation 8. Soda cabinets to be in good repairThis establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.
