To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Noted an RPZ backflow device in the produce rooom that no longer is connected to anything. Provide an RPZ backflow device (ASSE 1013) upstream of the misting system for the produce case. Provide a funnel drain below the RPZ backflow device and route to a floor sink. 2) Provide an ASSE 1055A air gap for the produce maxx at the prep sink. Flooring is worn to aggregate and areas of the concrete flooring do not bead water. Provide a water bead sealant. 2) Replace the FRP near the mop sink area. FRP was in disrepair. Provide a 3/8” cove base along the bottom of the FRP near the mop sink. 3) Clean out the floor sinks underneath the produce cases. Correct By: Correct Prior To Reinspection
