Final Construction inspection conducted with site superintendent. Construction completed and approved.Inspection report emailed to Jeremy S.Of Note:Establishment has forgone installing a soda fountain machine and will sell bottled beverages. A minor remodel will be needed to install soda dispenser in the future. The backflow will need to be an ASSE 1013 in lieu of the Double Check valve assembly at the mop sink. All non-carbonated beverage systems will need an ASSE 1022 upstream if installed as well. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Provide HUB drain for the dipper well so that it has a 1-inch air gap minimum with drain.2. Ensure all hot water is available to all sink fixtures within 45-seconds.3. Ensure hot water is provided to hand sinks between 100°F-110°F through mixing valves.4. Provide an ASSE 1013, reduced pressure zone assembly upstream of soda carbonator.5. Certify the backflow preventer and provide copy of the backflow preventer test certification.6. Ensure that all non-carbonated beverage systems connected to water supply have an individual ASSE 1022 upstream of the devices (coffee, beverage dispenser).7. Provide a rigid drain line connected with a minimum 1-inch air gap to the floor sink for warewash machine.8. Brace all waterlines and syrup lines above drain lines.9. Provide uni-strut for the drain lines and secure all uni-struts to the floor.10. Continue to fill all gaps, cracks, and penetrations (plumbing and electrical).11. Provide escutcheon plates and cleanout covers where needed (cleanout needs adjustment for back cook line, 3-comp hand sink to small).12.
