To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Stopped by to check on the status of the project. At time of inspection no significant progress has been made. On previous inspection spoke with site project manager John from 11 Western Buildings. At that time inspector sent inspection report to Billy Venia (PIC listed on application) requesting for an extension prior to 8/11/23. No extension was applied for.The approval of plans and specifications shall lapse and become invalid one year from the date of approval unlessa substantial portion of the work described in the plans and specifications has commenced by such anniversary date. No such work has been completed. Permit is closed, you will need to reapply for new permit.Inspection report emailed to Billy V.No county legal action will result from the inspection.Update:PM contacted permitting services and paid for 6 month extension. Please keep contact with inspector during the build process and call for 20% plumbing inspection when ready.
