PRIORITY VIOLATION-5-402.11, P: Backflow Prevention; 1. The three compartment sink is directly connected to the sewer system and is the only sink available for washing vegetables and thawing foods. An additional, NSF approved food prep sink that has hot and cold water and an indirect drain pipe that empties into a floor sink must be installed. There must be a minimum 1 in. air-gap between the end of the drain pipe from this sink and the flood rim of the floor sink that it drains into. 2. The soda fountain nearest to the drive through window has a drain pipe from the ice bin that connects to the soda waste pipe. This pipe runs into the floor then up out of the floor before emptying into a floor sink next to the ice machine. This soda fountain must be relocated so that the ice bin and soda waste can drain properly and indirectly into the floor sink with separate drain pipes from the ice bin and from the waste. 3. The salsa bar drain pipe is connected directly to the drain pipe from the self-service soda fountain waste pipe. There must be a minimum 1 in. air-gap between the drain pipe from the salsa bar and any connections to the drain pipe for the soda fountain waste. PRIORITY VIOLATION-5-402.13, P: Conveying Sewage; 1. The drain pipe for the three compartment sink leaks onto the floor from several spots. Repair all leaks in the three compartment sink drain pipe. Correct By: Correct Prior To Reinspection

Provide smooth, durable, waterproof and easily cleanable ceiling tiles to replace the acoustical ceiling tiles in these areas. Core-6-101.11, C: Surface Characteristics-Indoor Areas; 1. Remove all cardboard from the shelves in the kitchen. Core-6-201.11, C: Floors, Walls and Ceilings-Cleanability; 1.
