Observed Cooked chicken, pickled onions, and other TCS items in large reach in fridge with an internal temperature of 52F. Large reach in fridge found to have an internal temperature of 54F. All TCS foods improperly cold held were discarded by PIC at time of inspection. Establishment does not have adequate amount of cooling equipment. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct within 24 hours.

Observed cut lettuce, salsa, avocado, and pickled onions in cold table. Cut lettuce registered at 52*F with probe thermometer. PIC stated the lettuce was cut at 10am this morning. Embargoed 1lb of cut lettuce. Discussed options on maintaining cold holding with hot holding unit next to it. PIC stated intent to add a metal shield to block heat coming off unit as well as switching to shallow pans for all the items in the cold table. Ambient temperature of the rest of the cold table registered 37*F with min/max thermometer. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please address the following prior to next inspection: 1. Provide an ASSE 1013, reduced pressure zone assembly upstream of soda carbonator. 2. Certify the backflow preventer and provide copy of the backflow preventer test certification. 3. Reroute the drain line of the 3-compartment sink and prep sink to the floor sink. 4. Provide uni-struts or equivalent means to brace drain lines so they maintain air gap with floor sink. 5. Fill all gaps, cracks, and penetrations (electrical & plumbing). 6. Ensure hand sinks have metal splashguards as discussed. 7. Provide escutcheon plates and cleanout covers where needed. 8. Seal backsplash and sides of all sink fixtures and urinal. 9. Seal the hood vent system with heat resistant silicone (silver). 10. Ensure floor cracks and holes are repaired (smooth easy to clean). 11. Ensure delivery door is self-closing and add vinyl strip curtain. 12. Cleanout the floor sink of debris. 13. Provide plug for any floor sinks no longer in use. 14. Ensure all equipment is NSF/ANSI commercial and on holding proper temperatures. 15. Have the water heater unit serviced (unit is leaking too much water from the T&P relief drain). 16. Provide faucet with atmospheric vacuum breaker for mop sink. :
