Final Construction. Met with the contractor (Bobby). This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.Correct the following items: 1)Label the drain lines at the mop sink. 2)Provide a sign indicating the location, size, make and model of the overhead water heater. 3)Seal the gaps at th electrical panel. 4)Seal the gap at the 3-comp. sink/wall juncture. 5)Provide a metal splash guard on the left side of the kitchen hand sink. 6)Seal the sprinkler penetration in the walk-in freezer. Remove the ice build-up. Seal the side gaps at the base of the walk-in freezer door. 7)Paint all exposed wood under front cabinets and soda tower. Finish painting the interior of the equipment soffit boxes. 8)Seal cabinet plumbing penetrations and gaps at wall shelving. 9)Finish patching the epoxy floor at the front floor sink. 10)Install the soda tower and related equipment. Recommend relocating the chemical dispenser from the restroom to the mop sink. Provide a separate dedicated water supply line at the mop sink to provide water to the chemical dispenser. Call for a re-inspection when the items have been corrected. No County legal action will result from this inspection. Report sent via auto email.
