Final Construction inspection conducted with owner Angela C. Construction completed and approved. ***Permit is approved to use pre-washed and pre-cut cucumbers, tomatoes, lettuce, berries, canned chicken/precooked chicken. If establishment decides to alter their food process a prep sink with a minor remodel may be required by the Department. ***Permit is approved to do minimal prep: PB &J, cheese sandwiches, and chicken sandwiches. Inspection report was emailed to Angela C.Correct the following items prior to the next routine inspection:1) Current glass door refrigerator is not approved. Okay to keep food in this refrigerator until the new fridge arrives. New commercial grade NSF refrigerator is on order and will arrive mid-April 2024. 2) Flash the gap behind the 3-compartment sink. Use stainless steel.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Conducted onsite inspection to go over permit requirements. Per owner they have stopped selling any food/drinks. During onsite inspection the facility was not operating out of their kitchen. ***Establishment will be purchasing a limited amount of produce (i.e. cucumber) already pre-washed and cut from an approved source. Discussed maintaining receipts onsite. Establishment will be limited to their food/drink menu due to no prep sink. Project is approximately 95% complete.Correct the following items prior to final inspection:1) Provide a T-grid ceiling with washable vinyl ceiling tiles. Ceiling must cover the entire kitchen area.2) Provide an NSF approved hand sink with splashguards. Connect to hot and cold water and drain directly, or to the floor sink. 3) Provide an approved NSF/ANSI (ETL Sanitation) commercial grade, stainless front refrigerator. 4) Provide FRP behind the 3-compartment sink and mop sink. Ensure FRP is installed up to 12-inches past the drying racks.5) Install drying racks above the 3-compartment sink. 6) Provide self-closers to both restrooms.7) Provide rubber vinyl cove base to the toe kick of the cabinets. 8) Label the 3-compartment sink (wash, rinse and sanitize). 9) Provide a food manager’s certification.
