Final Construction inspection conducted virtually with site superintendent. Construction completed and approved. Inspection report was emailed to Chris S.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Conducted onsite inspection with Alexander R. Spoke with superintendent Chris via phone. Inspection report emailed to Chris.***If establishment preps any open food/drinks near the front prep table closest to the POS system a sneeze guard may be required.***Permit is not approved until final installation of the countertop is installed. Contact inspector once countertop is installed. Correct/address the following items prior to final inspection:1) Finish installing the countertop. Seal in all gaps underneath the countertops. 2) Remove the residential under counter refrigerator. Provide a commercial grade NSF/ANSI approved refrigerator. Residential equipment is not approved. 3) Provide the C of O.4) Silicone the gap to the left side of the 3-compartment sink, the gap in between the back kitchen hand sink and prep sink and between the wall and water heater stand.5) Repair the exhaust fan in the restroom. Equipment inspection conducted with Alexander R. At the time of this inspection, this project was about 85% complete. Inspection report was emailed to Alexander R.Correct/address the following items prior to final inspection:1) Observed a Turbo Oven with the capability to connect to a water source. Establishment does not have any water lines or floor sinks for this capability. Ensure water line and drain is capped. 2) Wrap the water stand with FRP and provide a 3/8” rubber vinyl cove base along the bottom of the stand. Seal the bottom of the cove base.3) Provide the C of O from the city.4) Provide at least 100’F of hot water to all hand sinks and at least 110’F of hot water to all other sink fixtures. Hot water must reach all sink fixtures within 45 seconds. 5) Install a splash guard to the left side of the prep sink.6) Provide a sneeze guard along the front countertop to protect any open food/drinks during prep. Sneeze guard must be 60 inches from the finished floor.7) Provide a 3/8” siliconized grout cove base along the wall and floor juncture in the front kitchen (near white tile) and the restroom. 8) Provide a dry well for the T&P drain line. Provide a p-trap or a fine mesh screen to prevent insects from entering the facility. 9) Provide a semi-gloss paint to all walls within the kitchen and restroom where there is no FRP.10) Seal in all pipe penetrations, and any gaps under the countertop once installed. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. Ensure water heater is 6-inches up off the floor. 4) Provide a splash guard to the left side of the prep sink.5) Provide a dry well for the T&P/drain pan off the water heater. 6) Finish grinding, polishing and sealing the concreate floors. Ensure all areas of the concreate floors in the restroom, front service and back kitchen area completely bead water. 7) Provide a semi or high gloss paint (light in color) throughout the front service area and kitchen.8) Ensure hot water reaches all sink fixtures within 45 seconds. 9) Provide a P-trap for the T&P/drain pan lines to prevent insects from entering the facility. Please continue with construction according to plans, stipulation letter, and inspections.
