This establishment is not participating in the award program at this time and had 1 Priority, 0 Priority Foundation violations on this inspection.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. Please provide a dual check valve that meets plumbing standard ASSE 1022 upstream of the beverage machines. One may serve multiple if the line is split after the backflow. Install in a accessible / visible location. If after a filter no debris screen is required upstream. Install downstream of the filter. Core-5-202.11 (B), C: Cleanable Fixtures - Drain grates are rusted out in several locations please replace or repair. PRIORITY VIOLATION-5-203.15, P: Backflow Prevention Device. Carbonator - Soda system backflow will need to be checked or a new valve installed upstream. You have three options, have the soda company replace the dual check valve on the carbonator, have the soda company inspect and sign off on the dual check valve on the carbonator and provide a tag on the unit, or install a reduced pressure zone backflow valve upstream of the soda system and install before the carbonator. Install with hub drain and lead drain to the floor sink. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained - Water softener media tank is leaking, please repair. Install a new cleanout cover at the mop sink on the side wall. Correct By: Correct Prior To Reinspection
Core-6-201.13, C: Floor and Wall Junctures, Coved, and Enclosed or Sealed - Please install coved base along the front of the beverage counters. Rubber base in the walk-in units is still in fairly good shape, when that comes loose replace with metal coved base and remove all the rubber base. Please fill in the sprinkler cooler ceiling penetrations where open. Polyurethane foam will work which also restores the insulation factor. Correct By: Correct Prior To Reinspection
