Observed walk in cooler thermostat showing high temperature on digital read out. Internal ambient temperature of walk in cooler was 43-45*F. Equipment must be maintained to keep food at 41*F or lower. Ice build up around fans in walk in cooler were observed. Repair staff for business came to site to defrost walk in cooler. Temperature control for safety foods were moved to other coolers on site. Reinspection to be completed within the next 24 hours

Observed that there was no Quat ammonia sanitizer residue at three compartment sink. Appears that system on site to add sanitizer to the three compartment sink is not correctly calibrated. Water was emptied and employee rinsed out sink and added new water and bleach to sanitize. Tested sanitizer water at 50 ppm. Employee added 2 1/2 caps of bleach to water in sink. Mechanical sanitizer mix to be repaired within 10 days. Re-inspection to be completed on or before Thursday February 20, 2025 Corrective Action: Correct prior to re-inspection.

PRIORITY VIOLATION-5-202.13 - Priority: Backflow Prevention, Air Gap Addl Notes: 1. The middle and right compatments of the 3 compartment sink are still directly connected to the sewer system. Provide an indirect drain pipe for these compartments that empty into the floor sink with a minimum 1 in. air gap between the end of the pipe and the flood rim of the floor sink. If the floor sink does not go through the grease trap provide a riser to the right side of the drain line instead. The riser must not go past the bottom of the 3- compartment sink basins. (SPS-EH-2012-014), (IPC §613.1)This establishment is not participating in the award program at this time and had 1 Priority, 0 Priority Foundation violations on this inspection.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2.
