Facility dumped all buckets, and used quat at 200 ppm. Ensure that quat sanitizer at 200-400 ppm or bleach at 50-100 ppm, or other effective means are used at 3 compartment sink and sanitizer buckets.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services.
