2ND CONSECUTIVE VIOLATION Observed a leak from the drainpipes of the 3-compartment sink. Also observed the faucet of the handwashing sink next to the pizza make-line to be leaking. The valve for the hot water was turned off to stop the leak. PIC informed the area supervisor to have the faucet repaired or replaced and will leave the hot water on to provide adequate warm water for handwashing. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed missing grout at the floor tile and wall juncture in the area in front of the walk-in cooler. Kitchen surfaces must be smooth, easily cleanable, durable and non-absorbent to deter pest harborage, pathogen growth, and soil accumulations. Corrective Action: Correct prior to next routine inspection.

Advised Manager that if food operation or menu changes, then a prep sink may be required.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Refill grout around floor tiles underneath dish area handwashing sink. Ensure grout level is flush to level of floor tiles. 3. Reseal gap around restroom handwashing sink. 4. Seal gaps at corners of hood above oven. 5. Seal gaps at top of corner coved base near oven. 6. Repair hole in floor tile below prep table area. Ensure repair is smooth and easily cleanable. 7. Repair top of metal coved base around inside perimeter of walk-in refrigerator that is bent away from wall. Ensure metal coved is sealed tightly to floor and wall of walk-in refrigerator. 8. Remove unnecessary chain hanging behind oven. 9. Reseal stainless steel plate to wall at left side of prep table, and seal gap at top of table to wall.
