Observed disclosure method missing on menu items requiring consumer advisory on Specialties menu, Ramen menu, Poke menu and Happy Hour menu. Menu items requiring disclosure on Specialties menu are "Truffled Salmon Tartar" (with soft egg and salmon), multiple specialty rolls with raw ingredients and "Chef's Selection" omakase-style serving raw sushi fish. Menu items requiring disclosure on Ramen menu are undercooked "Soft Egg" add-on item and "Super Tan Tan Ramen" (served with soft egg). Menu items requiring disclosure on Poke menu are "Soft Egg" add-on item, "Ahi Tuna", "Salmon" and "Yellowtail". Menu items requiring disclosure on Happy Hour menu are "Salmon Nigiri" and multiple specialty rolls made with raw protein ingredients. Also observed reminder statement missing disclosure asterisk on all menus. Offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Must provide approved consumer advisory at all points of sale prior to re-inspection on 04/21/25. Corrective Action: Correct prior to re-inspection.

Observed 0ppm of chlorine sanitizer in the bar. Operator primed drained, started a new batch and primed the machine. The chlorine sanitizer registered 50 to 100 ppm at time of inspection. Discussed with Person in Charge (PIC) the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Corrective Action: Corrected at time of inspection.

Observed several TCS items in the walk-in refrigerator to have date marks from 9/16 to 9/26. Discussed with PIC RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Embargoed the TCS items and PIC discarded at time of inspection. TCS items: homemade jalapeno ranch, garlic paste, roasted garlic, garlic in oil and roasted mushrooms. Corrective Action: Corrected at time of inspection.

Priority Foundation-3-501.15 (A), Pf: Cooling Methods. For a permanent fix to this violation, check out these simple tools in our Active Managerial Control Toolbox, made especially for you. https://www.maricopa.gov/4617/AMC-Toolbox---Cooling. >>>Observed deep container of miso broth and another deep container of soft boiled eggs at 60*F to 68'F cooling in the walk -in. Per PIC, the soft boiled eggs and the miso broth were cooked less than 2 hours prior to inspection and are in the process of cooling. Both items were transferred to the back reach-in refrigerator in shallow, uncovered pans to allow for proper cooling at the time of inspection. Also, observed in the ramen station at an ambient air temperature of 41*F, the Temperature Control for Safety (TCS) items with an internal temperature of 47*F 4o 51*F. TCS items are: roasted corn, butter, fried inari, soft boil eggs, grilled chicken, shoyu broth, miso soup and Paitan broth. Per Person in Charge (PIC) the food was prepped an hour before inspection and placed in the ramen prep table 1 hour before inspection. The TCS items were placed in the walk-in to rapidly cool to 41*F or less before being placed back into the prep table unit. Discussed proper cooling methods with the PIC. Provided helpful documentation. All items should be cooled in uncovered, shallow pans in the back reach-in refrigerator, by using an ice bath, or another effective means. Foods must be cooled from 135'F-70'F in the first 2 hours and from 70'F-41'F in the next 4 hours for a total of 6 hours. Correct By: Corrected At Time Of Inspection
