Observed establishment assembling pizzas, freezing, packaging, labeling, and then shipping them off through a third-party company. Discussed with PIC that this type of operating is operating out of the scope of their approved permit. A food production permit must be applied for if wanting to continue. Provided application information via email. Inspector will follow up as some information is under review.

Observed an employee finish a dish of cooked chicken liver but proceeded to finish off the dish by garnishing it with salt using their bare hands. Person in charge instructed the employee to place the cooked chicken liver back on the grill at the time of inspection. Discussed the importance of preventing bare hand contact with ready-to-eat (RTE) foods with the person-in-charge (PIC). Bare hand contact is not approved with ready-to-eat (RTE) food items. Corrective Action: Corrected at time of inspection.

Establishment is responsible for dating the past shellstock tags from when the last shellstock was sold from that batch. Reinspection will occur within 10 days. Instructed manager to put the sausage and mushroom back to walk-in refrigerator for fast cooling at time of inspection. TCS food must be cold holding at 41*F or below.
