Observed sewage actively backing up in the back of the establishment. Circular floor drain below the ice machine has a constant stream of dripping water entering from condensate pipe and is not draining. Standing water is present through the majority of the back half of the back room. PIC stated that a plumber has been out within the past week. Informed PIC that the operational permit will be suspended until the problem is eliminated. Suspension sign posted at time of inspection. Corrective Action: Correct prior to re-inspection.

Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Reinspection will be conducted within 10 days. Observed in house salsa cooling in a standing cooler but being covered. Salsa was still in the appropriate time/temperature for cooling. All TCS foods cooling were uncovered to allow for rapid cooling. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means.
