Observed staff store hot prepared food under cold foods on speed rack in walk-in refrigerator. All hot foods should be cooled on top shelves, separated from cold foods, to avoid temperature abuse of cold food. Observed small cold holding unit near service line with an ambient temperature ranging 45*F (bottom compartment) and 50*F (top compartment. Person in charge to place a work order and continue to hold shelf stable condiments only in unit. 4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Predefined Comment: All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection.

Live roaches observed in room where mop sink is located. Establishment is currently working on eradicating the issue. Person in charge provided pest control records with the most recent being 10/3, 9/30, 9/26 and 9/23 (Bi-weekly). Per PIC, establishment will be closing the kitchen on 10/9 to deep clean and will continue their bi-weekly pest control visits. Advised PIC to repair FRP walls and close off open holes on walls around the dishwasher area. Corrective Action: Correct prior to re-inspection. Establishment must be maintained free of pests.

Inspector's notes

Two containers of cooked pasta observed cooling with lids tightly sealed and the containers were stacked. The pasta was within proper cooling parameters. PIC took lids off and re-portioned the pasta into shallower containers to rapidly cool. Corrective Action: Corrected at time of inspection. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means.
