THIRD Consecutive. Observed exposed raw, porous concrete underneath broken tiles in various spots on the floor of the kitchen. Raw aggregate is exposed and holding water. Proper repair is required. Do not use unapproved methods. The establishment is to cover the raw concrete and aggregate with an approved surface or sealant to make the area non-absorbent, smooth, and easy to clean. Broken tiles are to be replaced. Discussed premises requirements with the manager. Corrective Action: Correct prior to next routine inspection.

Observed opened alternative milks (50-53°F), non-dairy whipped cream, and UHT single-serve creamers cold-holding above 41°F in the small refrigerator nearest the smoothie blending station. The unit measured 51.3 on the min/max thermometer, and the door was observed not to seal when shut fully. Alerted the manager, who discarded the opened alternative milk at time of inspection, and moved all foods to the nearby alternative small refrigerator. Discussed the importance of continuously monitoring cold holding temperatures. Corrective Action: Corrected at time of inspection.

The unit measured 51.3 on the min/max thermometer, and the door was observed not to seal when shut fully. Observed no paper towels in the handwashing sink nearest the fryer. Alerted manager who replaced the paper towels at time of inspection. Discussed maintaining soap, paper towels, and a trash basin near ALL the handwashing sinks. Time Temperature Control for Safety foods had been in the unit for over 4 hours and were embargoed, at time of inspection. The cooling unit measured at 49.4°F with the min/max thermometer. The manager placed a work order at time of inspection. The establishment has adequate refrigeration to operating safety. Manager will place TCS items in the nearby identical refrigeration unit on the other side of the aisle until the warm cooling unit can maintain temperatures below 41°F. The manager placed a work order at time of inspection. The temperature log book for 4 hour temperature checks was not filled out today and TCS foods in the unit over 4 hours were embargoed. TCS The establishment has adequate refrigeration to operating safety. Manager will place TCS items in the nearby identical refrigeration unit on the other side of the aisle until the warm cooling unit can maintain temperatures below 41°F. Discussed cold holding requirements. A reinspection will occur in 10 days.
