Observed at both hand wash sinks a mixing valve faucet dispensing hot water at 138*F. Discussed with person in charge (PIC) all hand wash sinks shall be set at a temperature that encourages hand washing. Per PIC the water temperature goes back down after approximately 20 minutes. Observed hand wash sinks at a temperature of 100*F after 20 minutes. Per PIC a work order was placed to correct the temperature to 100*F to 110*F. PIC presented work order to inspector at time of inspection. Corrective Action: Correct prior to next routine inspection.

Observed establishment using 4-hour time control for pico de gallo, diced tomatoes, cut lettuce, guacamole with tomatoes, mixed cheese blend and cheddar cheese on service line. Last recorded discard time observed 4pm, stocked more than 6 hours prior. PIC could not confirm if food items were replaced at 4pm. PIC voluntarily discarded all TCS food items in question at time of inspection. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Corrective Action: Corrected at time of inspection.
