Hand sink in the main kitchen is not bracketed to the wall properly. Any weight placed on it moves the hand sink. This is a hazard. Also observed at the same hand sink holes in the drywall below the sink. There is also no sealing around the hand sink. Please properly brace the sink to the wall, seal the edges between the wall and sink, and fill the holes under the sink by next routine inspection. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed ceiling tiles throughout the kitchen that are broken. Also observed a buildup of dirt and debris on some of the tiles. Please repair or replace the tiles so that they are clean. Also observed in the liquor storage room there is a vent with insulation sticking out. Please fix the vent/duct so that the insulation cannot fall onto bottles and boxes. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Of note: Observed multiple TCS foods in the walk in cooler with date marks extending beyond 7 days but still held within 7 days of preparation. Advised PIC to re-train staff to ensure TCS foods are dates with the prep date + 6 days. Advised PIC that prepped and frozen foods should be labeled with the prep and freeze date and marked again with the date it was removed from the freezer + 5 days, for a total of 7 days.

Observed multiple food containers wet stacked with visible food debris on them stored as cleaned. All food-contact surfaces must be cleaned to sight and touch. Observed slow drains in 3 floor drains on the cook like. Per PIC, establishment is actively conducting repairs on a collapsing drain line on the line. Observed a hand sink placed out of order until repairs are completed. No active backflows observed per PIC. All plumbing must be maintained in good repair at all times. Advised PIC to use nearby hand sinks and block off area until repairs are completed. Advised PIC to discontinue food operations if a backflow of sewage is observed. Corrective Action: Correct prior to re-inspection.
