Person in Charge (PIC) removed shut-off downstream from atmospheric vacuum breaker while onsite. Shut-off valves, including chemical dispensers, sprayer nozzles or y-valves, are not approved for continuous installation downstream from atmospheric vacuum breakers. All plumbing must be maintained in good repair at all times. Person in Charge properly connected chemical dispenser to dedicated waterline at time of inspection. Corrective Action: Corrected at time of inspection. Second consecutive violation. 5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Leak observed from faucet of 3-compartment sink at time of inspection.
