Inspector's notes

In the women's employee rest room, observed the plastic floor tiles falling off and harboring water, organic matter, and residues underneath them. The floor must remain clean, smooth, and easily cleanable. With the floor tiles falling off, it is not smooth or easily cleanable. They must be flat and sealed so water and debris cannot get under them. Instructed the establishment to find the source of water that is making its way under the tiles and prevent it. Please have these tiles repaired prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Core-6-201.11, C: Floors, Walls and Ceilings-Cleanability ---Observed baseboard in back produce room has come away from the wall. Manager put a work order request during the inspection. Ensure that baseboard is attached to the wall, and with coving on the bottom to keep water away from the wall. Correct By: Correct Prior To Next Routine Inspection
