Inspection report was emailed to Ron H.***The use of the F’real machine is not approved until all required documents for the variance have been received and approved by the Department (reference EHV-2355).NOTE: There is a dedicated water line underneath the front hand sink for the combi oven. Verified backflow device in the backroom is for the combi oven. Correct the following items prior to the next routine inspection:1) Provide a 3/8” rubber vinyl cove base to all areas of the kitchen and all cabinet bases as discussed.2) Provide a self-closer to the pizza spinner.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Ensure all hot water is available to all sink fixtures within 45-seconds.4. Ensure hot water is provided to hand sinks between 100°F -110°F.5. Provide an additional water line, with an angle-stop or ball valve, at the mop sink/ utility area to supply chemical dispensers.6. Provide uni-strut for the drain lines and secure all uni-struts to the floor as discussed.7. Provide a rigid waste drain line indirectly connected with a minimum 1-inch air gap to the sewer through a floor sink for all drip tray systems and equipment used for food service (No PEX Lines).8. Ensure that all concrete floors are finished smooth, and all expansion joints or cracks are properly filled. No open cracks or open expansion joints allowed.9. Fill all gaps cracks and penetrations (electrical and plumbing).10. Ensure all exposed wood is painted with several coats of semi-gloss paint.11. Plumbing Inspection. Walked with Paul. Project about 20% complete.Walked site and reviewed work done to date. For restrooms, air curtains cannot be tied in with lights in restrooms that are motion-sensor.
