Observed the back door to the kitchen propped open at the time of inspection. Discussed closing outer openings when not actively using to help prevent pest entry. Corrective Action: Corrected at time of inspection. 6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed rodent droppings throughout the back kitchen especially near the ice machine, soda bibs and dry storage room. Observed a wall hole behind the ice machine and a collection of material that appears to be insulation. Observed 20+ fruit flies in the back kitchen and near the mop sink. Observed back door propped open at the time of inspection. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Discussed cleaning all areas of rodent droppings to assist in monitoring. Advised PIC to placed covered traps near the back door and areas with high droppings. Advised PIC to place potential food sources such as flour, pasta, etc. into covered plastic containers to help eliminate food sources. Corrective Action: Correct prior to re-inspection.

Inside the cooler, observed TCS foods with internal temperatures ranging from 55-58*F when measured with a probe thermometer, including: meat balls, chicken meatballs, marinara sauce, pesto, mozzarella, soft-cooked bacon, butter, fresh mozzarella, and housemade cheese whiz. Except during preparation, cooking, cooling, or when time is used as a control all TCS foods are to be maintained at 41°F or less. PIC stated all foods had been in the cooler more than 4 hours and were voluntarily discarded at the time of inspection (see embargo). PRIORITY VIOLATION-5-202.13, P: Backflow Prevention, Air Gap ------ Observed a spray attachment at the 3 comp sink faucet stored approximately 6 inches below the flood rim of the sink. PIC stated the previous hose leaked and was replaced with a hose that was too long. Discussed that a minimum 1" air gap should be maintained at all times to prevent contamination of city water during potential backflow events. Discussed anchoring the upper portion of the spray arm to the wall to create a permanent fix. Discussed that a hook or some other means of storing the spray head may be used but must be stored above the flood line between use. An Active Managerial Control Intervention Visit was offered to the person in charge. Failure to correct repeat violations may result in legal action.A re-inspection will take place on 8/24/2023 to ensure priority foundation violation #36 has been corrected. Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits.
