Observed packaged chicken Souvlaki containing cooked chicken, potatoes, squash, zucchini, and red pepper. According to packaging the chicken souvlaki is prepared by Scratch Culinary Meal Prep. Contacted Scratch Culinary Meal Prep and was not able to obtain a valid permit with MCESD. Person in charge informed this product is considered an unapproved source and all food shall be obtained from sources that comply with law. Person in charge discarded product at time of inspection. Corrective Action: Corrected at time of inspection.

Observed packaged cooked ground beef tacos from RetroFit. No address, phone number, or consume by date observed on the label. Talked with Retro fit’s person in charge over the phone. The beef tacos were prepared on 5/12 and the date was marked on the label at time of inspection. Reviewed packaged food label requirements with the persons in charge. Corrective Action: Correct prior to next routine inspection.
