Final construction inspection for a Class 3 10+ E&D permit was conducted with Woody G. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Construction inspection for a Class 3 10+ E&D permit was conducted without management or construction team present. The facility was closed and not operating at time of inspection. The facility is approved to stock and train. The facility is not approved to operate. Please address the following items prior to the final construction inspection: 1) Ensure all coolers and refrigerators are on and measuring at 41' F or below. 2) Install an ASSE 1013 reduced pressure backflow preventer upstream of the RO system. There can also be no copper or copper alloys downstream of the RO system and backflow preventer. Certify and provide a test report. 3) Indirectly plumb the ice machine and customer soda fountain drains to the floor sinks and hub drains and provide 1" minimum air gaps. 4) Elevate the employee soda fountain indirect drains to provide 1" minimum air gaps above the floor sink. 5) Adjust the east drive-thru window to ensure it self-closes. 6) Caulk the ventilation hood gaps with a high temperature, aluminum silicone. 7) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. Please ensure the following items are addressed by the completion of the project: 1) Install ASSE 1013 reduced pressure backflow preventers upstream of the soda carbonators, reverse osmosis water filtration, and Combi-oven. Ensure no copper or copper alloy piping is downstream of the soda carbonator and RO filter backflow preventers. Certify and provide test reports. 2) Provide ASSE 1022 dual checks on each water supply line connected to non-carbonated beverage equipment. 3) Ensure all air curtain devices are provided with micro-switches. No manual switches can be utilized to operate the devices. 4) Ensure the 75 gallon AO Smith water heater is installed. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below.
