Final construction inspection for a Class 4 10+ E&D permit was conducted with superintendent Jack D. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. The facility is still approved to stock and train. At this time, all final connections for the brewery equipment will not be completed until a later date. The brewery area will be finalized under an additional minor remodel permit.Please address the following items prior to the final construction inspection: 1) The integrated pest management (IPM) that was submitted to the Department must be fully installed. This includes fly control measures, rodent bait stations, and bird deterrents. 2) The back charred wall area of the bar must be provided with a marine grade sealant or a clear matte epoxy. 3) Continue caulking all escutcheon plates and plumbing penetrations throughout the facility. 4) Provide an ASSE 1022 dual check backflow for the juice machine. 5) Provide a 3/8th inch minimum cove base to the floor/wall junctures of the mop sink room At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. E&D, class 4 10+Construction inspection was conducted with the superintendent, Ryan and Samantha. Construction is at 98% completion. The establishment can stock cold and dry products.Please address the following items prior to the final construction inspection:1. Pest control installation of bait stations and traps inside and outside of the facility must be installed before the final construction.2. Hot water must be available at point of service within 45 seconds upon activation. Provide hot water at 110*F for the 3-comp sink. Hot water was measured at 105*F with a thermocouple. 3. Tempered water must be at least 85*F.4. Repair/ replace/ remove the LFN9 backflow preventer installed on the sprayer arms at the 3-comp sinks in the kitchen and the brewery. They are leaking at the time of inspection.5. Noted the hi-temperature sanitizing dish washer is cold water fed. This is a conditional approval for this dishwasher. Provide a temperature recording log for the dishwasher. Log will be checked monthly for a year before final approval. Hot water fed will be required if the equipment cannot provide hot water at 180*F at the final rinse.6. Provide faucet for the work sink at the bar. 7. Provide rigid drainpipes for all drainpipes. This includes drainpipes of the brewery’s equipment.8. Remove the copper pipe downstream from the backflow preventer of the soda carbonator. Only PEX or stainless-steel pipe material can be used after the RPZ.9. Provide 1” airgap from the finished floor for all drainpipes from food/ drink equipment, work sinks, ware washing sinks and drip trays.10. Replace the door for the undercounter reach-in refrigerator inside the server area.11. Fill low grouts on floor and cove tiles.12. Finish the charred/ burnt wood panel walls and columns around and in the bar /food service area with several coats of marine grade sealant.13. Provide transitional strips between different floor level where needed.Additional power had been added for future air curtains if the facility fails to maintain the restaurant pests free. Cleaning procedure must be in place for kitchen, bar and dinning areas. Tables and areas around it will be clean up after each guest. Trash can must have self-closing lids. Doors must be closed during incremental weathers. An AMC/IMP plan does not mean the establishment can have pests inside the facility. An AMC/IPM plan is to go above and beyond to ensure pests are not attracted to the facility. Noted insects are already inside the kitchen and bar area.No County legal action will result from this inspection.Emailed the report to the owner and manager.
