Final Construction inspection conducted with owner Josefina A. Construction completed and approved. Inspection report was emailed to Josefina A.***Note that it is the owner's responsibility to obtain a Final or Certificate of Occupancy (C of O) from the city of Mesa.Correct the following items prior to reinspection on 12/7/23.1) Observed a new RO system. Provide a reduced pressure backflow device (ASSE 1013) upstream of the RO system. Do not install any copper or brass fittings or lines downstream of the backflow device. Only PEX, plastic and stainless steel is approved. Provide a funnel drain below the RPZ backflow device and drain to the hub drain or mop sink. Test and certify the backflow device and provide the test report. Or disconnect the RO system.2) Trim the riser below the 3-compartment sink so that it is below the basin.3) Relocate the filtered faucet to the prep sink.4) Provide a splash guard to the left side of the hand sink.5) Remove all unused items (ice machine) in front of the mop sink.6) Provide FPR paneling to the back side of the ice cream units.7) Provide a U-shaped toilet seat for the toilet.8) Repair the exhaust fan in the restroom. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2.
