Predefined Comment: Due to the violations noted on this inspection report, County legal action may result from this inspection.Predefined Comment: Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home. Observed walk in freezer containing boxes boiled corn, squash leaves, frozen bananas. Walk in approximately 25 by 10 feet and located on the main retail floor. Per person in charge the walk in freezer was installed one month prior to inspection. Remodel application must be submitted. Observed establishment with re-packed smoke fish. Currently the establishment has a class 2 retail permit that does not allow for re-packing. Person in charge has decided to stop repacking and will apply for appropriate permit if/prior to any repacking.Establishment has non-commercial stove in the back room. No cooked open food observed stored or sold at establishment. Per person in charge the stove is for personal use only. Establishment cannot cook or serve open food with current permit or equipment.
