Observed facility was without hot water and reached 76*F at the mopsink, the bathroom sinks and the breakroom sink. Discussed with person in charge (PIC) hot water of: 100°F is required at all handwash sinks; 110°F at all other sinks; 120°F at mechanical warewashing units sanitizing with chemicals; and 140°F at mechanical warewashing units sanitizing with high temperature. Observed previous walk-in/freezer removed from facility & a newly constructed walk-in/freezer has been installed in facility. Per person in charge (PIC), walk-in cooler was recently built a few weeks prior to inspection. Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.

Observed multiple missing cove bases at walls and floor junctures throughout the facility. Replace and repair prior to next routine inspection. Observed paint on CMU block walls in disrepair. Repaint with several coats of semi-gloss paint prior to next routine inspection. Observed missing door sweeps and weather stripping on the exterior doors at time of inspection. Replace door sweeps prior to next routine inspection. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Detail clean all surfaces, repair/ replace as needed. 2. Noted cove base is missing at walls / floor junctures This includes cove base in dry storage areas. 3. Noted paint on CMU block walls are worn. Please repaint with several coats of semi-gloss paint. 4. Please ensure all food/ drinks must be stored at least 6 inches off the floor and recommend 12 inches from the walls for pest control monitoring. 5. Inspection was conducted with the project manager, Joe. The establishment is open and operating at the time of inspection. Permit is approved to operate.No County legal action will result from this inspection.You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) "No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code." County legal action may result due to violations observed during this inspection. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections. This establishment/operation has demonstrated sufficient compliance to proceed with the permit issuance. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.

Corrective Action: Correct prior to next routine inspection. The floor sink outside of the tandem walk-in cooler/ walk-in freezer unit is not draining actively. Repair/ replace the floor sink and its drainpipe so the condensation liquid can be drained properly.
