Inspector's notes

Observed in use automatic chlorine dishwasher reading at 0ppm final sanitizer concentration, after several cycles were ran. Manager called for service at time of inspection, and 3 compartment sink is set up and available to use for sanitizing wares until machine is serviced. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Ensure chlorine is maintained at 50-100ppm in dishwasher. Corrective Action: Corrected at time of inspection.

Inspector's notes

No employee with supervisory and management responsibility with the authority to direct and control food preparation that is deemed a certified food protection manager (CFPM) was present at time of inspection. A CFPM is required to be onsite during the hours of operation. Please obtain certified food protection manager certification and keep certification accessible for health inspections. Corrective Action: Correct prior to next routine inspection.

Inspector's notes

Roll-up garage window observed propped open at time of inspection with no effective means of protection from entry of pests. Outer openings of a food establishment shall be protected against the entry of insects and rodents by filling or closing holes and other gaps along floors, walls, and ceilings; Closed, tight-fitting windows; and Solid, self-closing, tight-fitting doors, air curtains or alternative, effective means for protecting the outer openings. No application/ approval for alternative, effective means for protecting the outer openings on file. Reviewed requirements for approval for alternative, effective means for protecting the outer openings. Observed 2 rodent bait stations and no pest control devices installed. Instructed PIC to maintain roll-up garage closed until alternative, effective means for protecting the outer openings is approved. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed toilet room doors not self closing. Except where a toilet room is located outside a food establishment and does not open directly into the food establishment such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight fitting and self-closing door. Corrective Action: Correct prior to next routine inspection.
