This inspection was conducted with the manager on duty.A PDF copy of this inspection report was emailed to Nathaniel J. at the time of this inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the permit issuance. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.

1. The RPZ backflow prevention devices for the water supply lines to the soda fountain carbonator and to the bread proofer/oven must each be tested and certified. Corrective Action: Correct prior to next routine inspection.

Inspector's notes
1. Repair the cove base tile that is peeling away from the wall at the floor/freezer wall juncture on the inside of the walk-in refrigerator. 2. Repair the broken trim between the tile wall and the left side of the stainless steel sheet under the cooking hood. 3. Repair the broken cabinet door that has fallen completetly off of the cabinet beneath the self-service soda fountain. 4. Re-attach, re-caulk and seal the backsplash of the hand sink, adjacent to the food prep sink, to the wall. Corrective Action: Correct prior to next routine inspection.
