Observed no Quat sanitizer (0 ppm) dispensed at 3 comp sink since bag of sanitizer was empty. PIC had obtained new container of Quat sanitizer and manually adjusted concentration to 200 ppm at time of inspection. Informed PIC that Quat must be maintained at 150 ppm - 400 ppm per manufacturer. PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Observed an employee enter the facility to work for the day and put on gloves without a handwash and began to touch food contact surfaces such as deli paper used for making sandwiches. Employee completed a proper handwash before putting on new gloves and beginning work for the day. Proper handwashing is required between changing tasks and before putting on gloves. Employee discarded bread and completed a proper handwash, then remade sandwich using only the tongs and completed a proper handwash. Corrective Action: Corrected at time of inspection.
