To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.

Inspector's notes
In two separate glass front refrigerators, cheese and meat and sandwiches prepackaged food products which state 'Requires Refrigeration' on the packaging with an automatic lock missing on doors. Additional home style refrigerator with storage of TCS foods in storage room. Discontinue storing of back stock of TCS foods in refrigerator without automatic door lock. Provide automatic locks to two glass front refrigerators that store TCS (time/ temperature control for food safety) foods added or remove the refrigerated food products prior to permit approval. Corrective Action: Correct prior to re-inspection.

MCEHC Chap. 8, Sec. 2, Reg. 6g - All Micro Market display units offering time/temperature controlled for safety food shall be equipped with a self-closing door, an automatic shut-off device, and shall comply with all applicable regulations of this code as determined by the Department.
