Observed no recent commissary log. Corrective Action: Correct prior to next routine inspection. MCEHC Chap. 8, Sec. 3, Reg. 5c - Mobile food establishments shall provide a commissary agreement to the Department upon request, at permit issuance or renewal, and as a part of the documents submitted during plan review for a new permit. Ice cream vendors that only sell or dispense individually packaged, commercially processed items are exempt from providing a commissary agreement. Addl Notes: Commissary agreement is expired and not recent. The commissary listed has closed. Please provide a new commissary agreement and upload to the associated permit. Corrective Action: Correct prior to next routine inspection.

MCEHC Chap. 8, Sec. 3, Reg. 5e - Mobile food establishments serving time/temperature control for safety foods shall maintain a log of visits to the approved commissary or food service establishment as required by this Department. This log shall be maintained using a Department approved format and maintained with the establishment at all times. The logs shall be maintained for a period of 30 days and should be available for review by the Department.

Talked to manager about propane tank use in the vehicle. Manager wants to license another type II vehicle for drinks service in the future.

Observed open container of raw meat stored in tall fridge above ice used for drinks. Manager removed the meat and put on the lowest shelf. Ensure that raw meats are stored below ready to eat items. Corrective Action: Corrected at time of inspection.

Observed facility had a table set up outside, on the ground, with a customer self service, salad bar type set up with veggie containers with scoops for customers to help themselves. Talked to manager about employees adding the foods to orders, and the food being located inside the vehicle. Can have cooler with bottled drinks, or bottled sauces outside. ENsure all food preparation is done inside the vehicle at all times. Corrective Action: Corrected at time of inspection.

Mobile truck not present during time of inspector's arrival at 7045 E Main St. Inspector contacted the operator via email on 8/9 to confirm business hours. Inspector called Operator on 8/12, voicemail box was not set up. Operator called inspector back and stated they were out of operation for the last month due to high temperatures outside. They will be operating again next week.
