Observed missing concrete around the drain for the mop sink in back rooms. Water is pooling in this area. Also observed concrete floor surrounding floor drain near back area walk-in freezer and dairy cooler in disrepair and accumulating pooled water. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. Repeat violation: 3rd consecutive.
