Observed Holes in walls and missing Ceiling Panels in back kitchen. Raw wood panel is covering ceiling access - replace with a smooth cleanable panel. Hole in wall next to hand sink (at cooks line) is open from plumbing repair and needs to be closed with a smooth and cleanable surface. Back kitchen surfaces and floors have build up of soil/debri. Discussed with PIC a regular detailed cleaning schedule and walk thru on a more frequent basis. Corrective Action: Correct prior to next routine inspection. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection.
