There was a chemical dispenser line attached to the mop sink faucet at time of inspection. This increases the risk of chemical contamination going into water supply. Person in charge (PIC) removed the line at time of inspection. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed that the tile on the wall near the ceiling in the kitchen is buckling off the wall. Also observed water staining and damage to ceiling tiles throughout the kitchen and back-office area. Some of the surfaces look like there is organic matter on and around them. The ceramic wall tiles and the ceiling tiles need to be easily cleanable to sight and touch. They need to be repaired in order to be easily cleanable. Physical facility must be maintained in good repair at all times. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed two buckets in the kitchen, behind food prep stations collecting water that is leaking from the ceiling. Person in charge and employees stated the leaking is an ongoing issue 2-5 months and is worse after raining. Leaking from the ceiling increases risk of food contamination. Discussed with person in charge that if leak gets worse or falls onto food prep station to call Environmental Health Service to access the risk while repairs are being made. Corrective Action: Correct prior to re-inspection.
