Observed a stream of water running out of the wall behind the ice machine. The water appears to be from a leaking pipe inside of the wall, which is flowing out from the bottom of the FRP and top of the cove base tile on the wall. According to PIC, a work order has been created. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to next routine inspection.

3RD CONSECUTIVE VIOLATION Observed an accumulation of soil and debris on the floor behind and under the ice machine and adjacent cook-line equipment. Discussed cleaning the area. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Corrective Action: Correct prior to next routine inspection. 6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed areas of missing grout in between the floor tiles in the front of the kitchen, some areas of the cook-line, and in the ware washing area. Water and debris observed to be in the voids between the tiles. According to PIC, estimates are being done to repair the floor. A smooth, easily cleanable floor surface will allow water to properly drain, thereby preventing pooling which could attract pests such as insects and rodents or contribute to problems with certain pathogens such as Listeria. Corrective Action: Correct prior to next routine inspection.

Inside the unit was 4 containers of coffee with cream at 49*F; cream at 49*F; container of 8 children milk at 49*F; whipped cream bags (eight) at 53*F. Notified Person In Charge (PIC). PIC confirmed internal temperatures. Last check of unit was at 2pm. Time of inspection is 4pm. Directed PIC to move all listed tcs food items to main walk in to cool to 41*F. Confirmed 30 minutes later. ======Except during preparation, cooking, cooling, or when time is used as a control all TCS food to be maintained at 41°F or less. Corrective Action: Corrected at time of inspection.
