Observed multiple live German cockroaches in the back of house at time of inspection. Establishment must be maintained free of pests, and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. 2nd repeat violation. Observed slimy organic matter accumulating on the deflector panel of the ice maker. Ice maker shall be cleaned as often as necessary to prevent any accumulation of soils or organic matter. Please have ice maker cleaned and cleaned more frequently. Cook informed the lettuce had been over the ice earlier in day and placed in reach in unit. Observed a large plastic container containing shredded lettuce cold holding at internal temperature of 70 deg F in a reach in unit holding >41 deg F. Cook informed the lettuce was placed in cold unit a short time prior and placed in alternate cold unit. This is the second consecutive violation. Corrective Action: Corrected at time of inspection.

Observed cooked chicken breast in the cooler with an internal temperature of 44*F-46*F using probe thermometer. Person in Charge (PIC) stated the chicken had been cooked and placed in the cold holding unit the night prior, before closing. Discussed with PIC the importance on maintaining TCS foods at 41*F or lower. PIC voluntarily disposed of the chicken at time of inspection. See embargo entry. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. All TCS foods improperly cold held were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.
