Due to the violations noted on this inspection report, County legal action may result from this inspection.Observed establishment demolish existing wall and expand food operations into neighboring suit and added a new walk-in cooler. Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.

Observed no hand drying provision at bathroom handwash sink. Handwash sinks shall be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities shall be maintained in a condition that promotes handwashing. Person in charge (PIC) provided hand drying provision for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Observed can of raid insecticide spray in 3-compartment sink room. All pest control for the establishment shall be performed by a certified pest control specialist and chemicals for pest control shall not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.

Observed accumulation of organic matter in floor drain below ice machine and observed dust/food debris behind soda machine. Physical facility shall be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Establishment shall clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. Repeat violation: 2nd Consecutive. 6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed holes in drywall along floor behind employee bathroom toilet and missing tiles underneath ice machine. Physical facility shall be maintained in good repair at all times. All floors, walls, and ceilings shall be maintained in good repair and repaired such that it is smooth and easily cleanable. Per PIC, establishment is in process of soon repairing walls and broken tiles. Repair holes in wall prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Person in charge (PIC) instructed to remove and store packaged alcohol from bathroom in a way that prevents contamination. PIC began to relocate packaged alcohol.
