In multiple areas throughout the kitchen, observed partial or entire missing floor tiles, exposing the concrete underneath and collecting water. PIC stated they would put in a work order. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection.Also observed an accumulation of grease deposits and food debris on the walls and ceilings of the prep area and warewashing area. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Ice Cream mix observed with internal temperature of 46*F while being cold held inside the bottom half of the soft serve machine. The ambient temperature of the unit was 47*F according to inspectors min/max thermometer (after 15min inside the unit). It was unclear as to how long the ice cream mix was held at that temperature. Discussed the temperature danger zone (range between 41*F and 135*F) with PIC. PIC voluntarily discarded the product at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Corrected at time of inspection. Establishment has another soft serve machine to use while awaiting repair.

Observed organic matter build up on soda nozzles. PIC disassembled soda nozzles and washed, rinsed, and sanitized nozzles at time of inspection. Discussed with PIC and employees that the soda nozzles must be cleaned and maintained at a frequency as to prevent future build up. Corrective Action: Corrected at time of inspection.
