Inspector's notes
Observed an abundance (30 +) of dead flies inside the electric wall trapping device in back prep room and a larger insect stuck in the grating of the same device. Observed a hanging sticky fly sheet to the left above the hand sink with an abundance (20 +) of small insects on it. Observed large dead roach in the floor drain underneath the far most right counter where storage is. Observed floor trapping devices underneath the front counter where storage is, a few were clean, and others had dead insects. PIC was informed of the dead insects. Explained to PIC that the devices are allowed, but they need to be monitored frequently enough to dispose of dead insects, as to not attract more pests. Corrective Action: Correct prior to next routine inspection.

Observed small drain-pipe from the ice cream machine draining directly into the floor drain. The pipe is 2-3 inches inside the drain, an air break is present. Explained to PIC that the pipe needs to be moved to cut so that there is a one-inch air gap between the end of the pipe and the top of the drain. Discussed the importance of preventing backflow and possible bio-film growth into equipment, as well as being able to properly clean the floor drains. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed food and other debris underneath the ice cream chiller, as well as behind ice cream machine and equipment in the back prep room. Observed yellow/orange discoloration on the back right wall and ceiling above the waffle makers. Observed a build-up of grey material on the air vents in the front service area. Informed PIC that these areas need to be cleaned and continue to be cleaned at a frequency which prevents the accumulation of debris and organic matter. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection.
